Participants will learn not only how to use these tools, but also when each is most effective.
Real-world examples, such as a 5 Whys analysis, will illustrate how root causes emerge through systematic questioning. This session also introduces the human side of RCA, including cognitive biases, assumptions, team dynamics, and the common pitfalls that cause organizations to stop at surface-level explanations.
By emphasizing collaboration and system-level thinking, participants will learn why 90% of perceived human errors can often be attributed to system errors or deficiencies. Part 2 prepares attendees to conduct RCA with greater accuracy, consistency, and cross-functional engagement.
